After you have completed the application, it will need to be printed and signed. Then mail the signed application and a check for $650 to:

When you have completed the application, click Notify and Print. At this point, the tournament directors will be notified that you have filled out an application. When your paper application and check have been received, your team name will appear on the ‘2009 Team Confirmation’ Page.

*  You must sign the paper application before you mail.  *

Please keep in mind that your roster form must be completed by May 30, 2009 to make it in the tournament scouting booklet. Any rosters received after this date will not be handed out !
